Community Referral

Community referral (or social prescribing) connects you to local support for emotional, social, or practical issues. Trained professionals offer free, confidential, one-to-one support, helping you overcome challenges and access community services to improve your health and wellbeing.

How to Access Community Referral

Many GP practices offer their own community referral service, and you can easily access this by requesting a referral from your GP practice.

If your GP practice doesn’t have this service, don’t worry! You can still access city-wide support through our Community Support Workers, who are available to provide advice and assistance on a wide range of non-medical matters, as long as you’re not currently receiving social care.

Mark, 36

Mark was struggling with growing debts due to gambling and feared losing his home. After speaking with his GP, he was referred to a link worker. They worked together on a budgeting plan, a full benefits check, and found professional help for his gambling. Several months later, Mark has regained control of his finances and is planning for the future.

 

Julianne, 48

Julianne struggled with her weight and low confidence, which affected her social life. Her link worker helped her explore activities she enjoyed, like dancing, and found a nearby dance fitness group. The link worker joined her for the first session, and three months later, Julianne is a regular participant and has built new social connections, boosting both her confidence and health.
