At Porter Brook Medical Centre our Social Prescribers can support with things such as housing, benefits, debt, healthy lifestyles, social networks and employment.

We can work with ages 16+ and offer either telephone or face to face appointments at the practice. 

We can work with people for up to 8 weeks, supporting you to work on your goals and connect you into services and organisations in your community.

• Ask at reception for a referral or
• mention it next time you see a GP or Nurse.
